Access personalised support within St John’s Hospital with our dedicated team.

Being in hospital or caring for someone who is in hospital can be worrying, especially if you are a new carer.

If you, or the person you care for, is in hospital, support is available to help you plan the next stage of your care.

Our dedicated hospital team will work with you and hospital staff to give you the best possible chance of a successful discharge from hospital.

We’ll make sure you know your rights, you’re involved in the discharge plan and are in control of ongoing support when you’re back home.

How can we support you and your loved one through the hospital journey?

Our hospital team will ensure:

  • You have an understanding of hospital discharge procedures

  • You know how to be involved in meetings with an understanding of plans going forward

  • You feel ready and confident for your cared for person to be discharged from hospital

  • You have an opportunity to chat to our staff in the hospital through one:one and drop in sessions

St John’s Hospital Drop-in Session

Our team is available on the last Wednesday of every month, from 2–4pm, in the Information Hub at the main entrance of St John’s Hospital.

From time to time, we’re joined by hospital staff or local organisations who can offer specialist advice and support.
